Waiver & Agreement
I, the undersigned, hereby acknowledge and agree that my use of Hell's Gate Gym's facilities, equipment, services, and programs involves an inherent risk of injury. I voluntarily assume all risks of injury, illness, or damage, including death, which may arise out of my use of Hell's Gate Gym's facilities.
I agree to waive, release, and discharge Hell's Gate Gym, its owners, agents, employees, and volunteers from any and all claims, demands, injuries, damages, actions, or causes of action arising out of my use or intended use of the facilities.
I have read and fully understand the above release/waiver and I voluntarily accept its terms.
